Field Trips Checklist
Initial Steps
- Review Academic Field Trips and Driver Guidelines
- Collect signed Informed Consent and Hold Harmless Release Forms from each student who plans to participate and deliver to department office. Be as specific as possible identifying risks.
- Fill out Academic Field Trip Participant List. Each student is to list his/her emergency contact person’s name and phone number. The original of the list should be left with the department secretary.
- Collect Authorization to use Privately Owned Vehicles on State Business (Std 261) if students are being allowed to drive personal vehicles to the destination.
The Day of the Trip
- Review with the group CSU Use of University and Private Vehicles Policy Guidelines paying particular attention to the prohibition against transportation of alcoholic beverages or other controlled substances.
- Review Accident Reporting Procedures and provide each driver with an accident packet available from the Motor Pool supervisor.
- Take a copy of the Academic Field Trip Participant List.
